Chapter 30: Montgomery GI Bill

The following are needed for all Active Duty, Separated, or Retired Chapter 30, Chapter 30/34, Chapter 30/32 or Chapter 32 students.

  1. Enrollment Certification Request - Needed for each semester.
  2. Program of Study.
  3. Veteran's Accountability Statement.

Important Notes:

If a Certificate of Eligibility is received, do not fill out VA Form 22-1990.

Or,

If application was done electronically at VONAPP (Veterans On Line Applications) website, a confirmation page must be obtain. VA Form 22-1990 is not filled out.

Are you currently on active duty with the Air Force, Army, Coast Guard, Marines or Navy?

  1. Are you a new student who has never used VA benefits before?
    - VA Form 22-1990: Application for VA Education Benefits.
    - The form must be signed by Base Education Officer.
  2. Have you used VA benefits before at another college?
    - VA Form 22-1995: Request for Change of Program or Place of Training.
    - The form must be signed by Base Education Officer.

    - VA Form 22-1995: Request for Change of Program or Place of Training.

Did you enlist before January 1, 1977? Did you have remaining entitlements under the Vietnam Era Veterans Educational Assistance Program ( Chapter 34 )?

  1. Are you a new student who has never used VA benefits before?
    - VA Form 22-1990: Application for VA Education Benefits.
    - DD Form 214: Member 4.
    - VA Form 21-686c.
    - Birth Certificate(s) for dependent child(ren).
    - VA Form 21-674. If dependent child(ren) is over 18 and enrolled in school
    - Current marriage license.
    - Divorce decrees - final judgment (from all prior marriages of the Veteran and the current spouse).
  1. Have you used VA benefits before at another college?
    - VA Form 22-1995: Request for Change of Program or Place of Training.

Did you convert from VEAP program Chapter 32 to Chapter 30?

  1. Are you a new student who has never used VA benefits before?
    - VA Form 22-1990: Application for VA Education Benefits.
    - DD Form 214: Member 4.
    - DD Form 2366 (signed at Finance upon discharge “out-processing" electing to use Chapter 30 instead of Chapter 32 VEAP).
    - Proof of $1200 contribution, copy of LES or other documentation.
  2. Have you used VA benefits before at another college?
    - VA Form 22-1995: Request for Change of Program or Place of Training.

Chapter 32 VEAP - Post Vietnam Veterans' Educational Assistance Program.

Did you enlist from January 1, 1977 to June 30, 1985?

  1. Are you a new student who has never used VA benefits before?
    - VA Form 22-1990: Application for VA Education Benefits.
    - DD Form 214: Member 4.
    - If DD Form 214 Block 15a is yes - No Proof of $1200 contribution is needed.
    - If DD Form 214 Block 15a is no - Proof of $1200 contribution is needed. (Copy of LES or other documentation.)
  2. Have you used VA benefits before at another college?
    - VA Form 22-1995: Request for Change of Program or Place of Training.

Chapter 31: Vocational Rehabilitation Program

Do you have a service-connected disability rated at 10 percent or more from the VA that entitles you to disability compensation? Has the VA Form 28-1900 Application been accomplish (Application can be completed at the Ft .Walton Beach Voc Rehab Office)?  Do you have an Individualized Program Plan with VOC Rehab?  

  1. Chapter 31: Vocational Rehabilitation.
    - VA Form 28-1905: Authorization and Certification of Entrance or Reentrance into Rehabilitation and Certification of Status. NOTE: The student must receive this document from Ft. Walton Beach VA Voc Rehabilitation and Employment office.
    - Enrollment Certification Request - Needed for each semester.
    - Program of Study.
    - Veterans Accountability Statement.
  2. Returning Chapter 31: Vocational Rehabilitation
    - VA Form 28-1905: Authorization and Certification of Entrance or Reentrance into Rehabilitation and Certification of Status. NOTE: The student must receive this document from Ft. Walton Beach VA Voc Rehabilitation and Employment office.
    - Enrollment Certification Request - Needed for each semester.

Chapter 1606: Selective Reserve Educational Assistance Program

Are you currently serving in Selected Reserve?
NOTE: This is not inactive reserve status which is different from selected reserve.

If a Certificate of Eligibility is received, do not fill out VA Form 22-1990.

  1. Are you a new student who has never used VA benefits before?
    - VA Form 22-1990: Application for VA Education Benefits.
    - DD Form 2384-1: Notice of Basic Eligibility (NOBE).
      NOTE: The student must receive this document from their Reserve Unit.
    - Enrollment Certification Request - Needed for each semester.
    - Program of Study.
    - Veterans Accountability Statement.
  2. Have you used VA benefits before at another college?
    - VA Form 22-1995: Request for Change of Program or Place of Training.
    - Enrollment Certification Request - Needed for each semester.
    - Program of Study.

Chapter 35: Dependents and Survivors Education Assistance Program

Are you a (surviving) spouse and/or child of a veteran with a permanent and total service (100%) connected disability or whose death was service connected or who died of any cause while rated as having a permanent and total service (100%) connected disability? Payment cannot be made to dependents or survivors without the veteran's file number and a VA assigned suffix code.

If a Certificate of Eligibility is received, do not fill out VA Form 22-5490.

Are you a new student who has never used VA benefits before?

  1. Are you a Dependent Spouse?
    - VA Form 22-5490: Application for Survivors' and Dependents' Educational Assistance.
    - Enrollment Certification Request - Needed for each semester.
    - Program of Study.
    - Veterans Accountability Statement.
  2. Are you a Dependent Child?
    - VA Form 22-5490: Application for Survivors' and Dependents' Educational Assistance.
    - Birth Certificate.
    - Enrollment Certification Request - Needed for each semester.

Transient VA Student

Are you a transient student from another college or university using your VA benefits?
To be considered for transient status, a student must be enrolled at Northwest Florida State CollegeC as a "non-degree seeking" student and enrolled in an approved program at their primary institution. To begin certification at NWF State College an approved "Transient Student Form", listing the courses needed at NWF State College that will be accepted towards the veteran's degree program at their primary institution, must be on file in the NWF State College Veterans Affairs Office

  1. New Transient VA Students
    - For each Semester a New Transient Form from the Parent School is required.
    - Enrollment Certification Request - Needed for each semester.
    - Veterans Accountability Statement.
  2. Returning Transient VA Students
    - For each Semester a New Transient Form from the Parent School is required.
    - Enrollment Certification Request - Needed for each semester.

Other Miscellaneous Information

  1. Advance Pay
    - If Veteran wants Advance Pay the VA has to have all paperwork at least 45 days prior to the first day of school.
    - Veterans must go to Niceville Campus VA Office to sign the request.
    - To qualify for Advance Pay the Veteran must be new or have had a break in enrollment.
    - Advanced Pay is never guaranteed.
    - The first check goes to Niceville, remaining checks are sent to the student.
  2. VA deferments
    - All VA paperwork is needed before a VA deferment is written.
    - The student must be a Veteran using their entitlements.
    - Only ONE VA deferment per academic year.
  3. Program of Study
    - All VA students must have a Program of Study on file before the second semester.
  4. Tuition and Fees
    - Active Duty is only tuition and fees only.
    - If a VA student is less than 1/2 time they will get only tuition and fees only, except Chapter 1606 students.
