Disclosure of a disability is voluntary. If you are a currently enrolled student with a documented disability and you need special services, accommodations or access, please follow these steps:
Step-by-Step Guide for Self-identifying at Northwest Florida State College
- After self-identifying as a Special Needs student, you will be granted appropriate accommodations within the classroom for that semester. If you wish to continue to receive support services in one or all of your classes for future semesters, please remember to complete the Request for Accommodations form each term in which services are needed and submit the form to the Counselor for Students with Special Needs. This form may be submitted electronically by clicking on the following link: (link inserted here). These forms are also located in the Academic Success Center on the Niceville campus, in the Student Services area of each of the other Northwest Florida State College sites, and on this website under Forms. Remember that the accommodations requested must be appropriate for the disability and previously agreed to by the Counselor for Students with Special Needs. If you feel that other accommodations are needed, please contact the Counselor for Students with Special Needs at (850) 729-6079 or smithe15@nwfsc.edu.
- Bring written documentation of your disability with you to the appointment. The written report must be current (within the past 3 years) and provided by a qualified professional such as a physician, audiologist, psychologist, psychiatrist, learning disability specialist, VA counselor or counselor for the Division of Blind Services. This information will assist the college in determining adequate, appropriate and reasonable accommodations.
- The documentation must state:
- Diagnosis with specific evidence of a disability. See Documentation Guidelines
- Test scores which support a cognitive or learning disability
- Medications and any adverse side effects
- Recommendations regarding necessary accommodations
- Provisional accommodations might be offered in the interim, but a final determination of accommodations will not be complete until your documentation is received by Disability/Special Needs Support Services.
Step-by-Step Guide for Receiving Accommodations in the Classroom

- It is important that at least ten (10) working days are allowed for special arrangements to be made.
- Contact the Counselor for Students with Special Needs prior to the beginning of each term in order to discuss any necessary changes in your accommodations.
- In the event you encounter any difficulty or other concern regarding your accommodations, contact the Counselor for Students with Special Needs immediately.
